I have also noticed that when they do release scales out on time that have QC issues.
It is a shame how Kurumi possibly has turned out, as she was a scale I am interested in. But I do agree that it is concerning that GSC hasn't said what the issue was.
On the nendo side, I'm 10/10 on recent nendos receiving having massive dirt spots or dust under the clear coat of faces. The recent Itachi one had a 2 mm+ size area of dirt/grease on the tip on the nose. If we can have cameras monitor quality control for food production in factories, you'd think GSC could maybe invest in a bit of the same for the assembly line for nendo faces. The faces are made on what is essentially an automated assembly line.
The QC issue with dirt and dust under the clear coats isn't just them. Even Alter is falling victim to it. If it isn't dust, then it's grease or splotchy paint/bubbles. I fear we're reaching the hyper commercialized point where it's quantity over quality now. I guess we can just be somewhat hopeful things will get better (especially if GSC is indeed planning to move some factory operations to Japan). Still sad I can spend a few dollars in hobby paint and repaint a PUP to look way better and cleaner than when I got it.

These last two of proto vs actual really seem off unless the picture is due to heavy overexposure. She looks like a pumpkin more than the reddish-orange of the prototype. The fact GSC didn't specify on the defect is concerning. Normally they're willing to admit there was a defect in a part or paint error. Guess an on-time GSC scale figure release was too much to ask for in the world of PUP taking all priority.

But if these pictures are to be believed as the final product...what's the defect? It seemed fine enough on first glance. Some angles, colors, and sculpts weren't quite the same, but it's odd they just said the whole product was TBD now. I don't doubt that they may have shipped it to some shops, but also bear in mind where these pictures came from and where the factories are. Wouldn't be a shock if early release factory copies were going out. This is where my problem with the timing of the "defect" discovery comes in. By the time they noticed and tried to recall, stores may have already had the figure and still tried to collect. If the buyer never checks GSC's news listings, then how would they know there was an issue?

I got a payment request from Ami Ami then a week later GSC announced his release date a week later. I think they may of shipped her early, it did happen with my Yuuta nendoroid.

Maybe there were complaints. The only reason this could be a "released" version is if GSC already shipped to shops and didn't realize the error until after. And considering some shops, they'd just go ahead and sell the defect without much thought. Delays do equal cancellations by impatient customers. How was this released if GSC said there was a problem and never shipped?
